Does it cost extra to add audio to my video design?

Adding audio to an image changes it to a video. Adding audio to a video is free.

Videos always cost $14.95 on free basic accounts*. The download costs $16.95 if it contains stock graphics by GettyImages. 

Videos with or without additional audio are completely free for PosterMyWall Premium and Premium Plus subscribers.
